Brief Job Description: The Care Representative provides clinical and administrative support for a single point of entry into services.

Perform telephonic screenings for individuals and families seeking to enter services.

Determine acuity based on telephonic screening and schedule clinical intakes accordingly.

Maintain accurate and timely schedule master.

Follow up on after-hours requests for new service.

Perform telephonic follow-up for missed appointments.

Coordinate with other local agencies to gain referral/client information as appropriate.

Assist Clinical Care Managers in clerical tasks as needed.

Qualifications:

Bachelor’s degree in a mental health related field.

Be eighteen years of age or older.

One year experience working with individuals who receive services for treatment of a mental health disorder, substance abuse disorder or co-occurring disorder preferred.
