Title: Targeted Case Manager

Hours Status: Full-Time
Pay Range: $15.87 hourly

Brief Job Description:  Responsible for providing case management services to individuals with substance use and mental health disorders. The caseload will consist of adult women and children. Duties include, but are not limited to: coordinating with LifeSkills’ staff and external providers to access services and resources to assist client’s. Additionally, providing training to community partners and pregnant women will be required periodically.

Qualifications include: BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences degree.  One year of experience working with SUD adults or one year of case management experience, post-graduation, or 6 months if individual possesses master’s degree in approved field and a valid KY driver’s license.
