Hours Status: Full Time – Monday - Friday (Flexible Work Schedule)
Pay Range: $15.87 Hourly
Brief Job Description: The Regional Case Manager is responsible for providing case management services to individuals in the State General Fund (SGF) program(s), developing and monitoring action plans designed to prevent service disruption or community displacement for individuals with intellectual and developmental disabilities; partnering with other agencies to coordinate and ensure person-centered planning circles for individuals at risk or in crisis; and providing service coordination when needed.
ESSENTIAL FUNCTIONS:
- Responsible for assuring that individuals rights are respected and individuals are free of any and all forms of abuse or mal treatment.
- Responsible for supporting individuals to maximize personal choice.
- Responsible for developing person centered plans based on individual preferences and needs using a person-centered approach.
- Responsible for coordinating individual supports as necessary and available.
- Responsible for proper and appropriate documentation of all supports provided in a timely manner.
- Responsible for documenting all incidents and accidents in a timely manner, on the required format, and notifying the appropriate parties
- Bachelor’s degree, Associate’s degree and/or equivalent experience in related field.
- Minimum one year experience working with persons with Developmental Disabilities preferred.
- Valid drivers’ license and proof of insurance.
- Knowledge of community resources and ability to develop/create appropriate support resources preferred.
