Brief Job Description:  Provide support services to youth and young adults, and their families, who have been identified as experiencing their first episode of psychosis. Duties include partnering with the youth and young adult, to the greatest extent possible, in assessing needs and barriers to those needs.  Collaborating with others directly related to youth and or young adult to assist with identifying and obtaining services, treatments and supports, and in preventing and managing crises. Making referrals to other services when needed. Providing educational and medical support to the youth and or young adult as well as other team members. All while working alongside of the youth and or young adult and the families and following applicable State, Federal and Medicaid guidelines.

  • Bachelor of Arts or Science degree in a behavioral science with one year of case management experience working with youth or young adults with mental health illness or one year of experience working with youth or young adults with substance use disorder. Experience must be post degree.
  • Successfully complete Case Management training within six months of employment. 
  • Have reliable transportation with valid driver’s license and auto insurance.
  • Possess strong written and verbal skills.
  • Be willing and able to work flexible hours to include some evening and weekends as well as alternating crisis phone responsibilities.
  • Criminal background checks to meet agency standards.
