Title: CASE MANAGER COORDINATOR- THERAPEUTIC FOSTER CARE

Hours Status: FULL-TIME, SALARY

Brief Job Description: This position will review DCBS and Private Placement referrals for appropriate placements and coordinate placements. Provide on-going support and coordinate training with existing foster parents. Perform recruitment efforts to obtain additional foster parents and assist in training new foster parents. Provide TFC training and supervision to TFC Case Managers. Provide case management services to children placed in the TFC program as overflow when other TFC Case Managers are over 12. Intensive monthly in-home and out-of-home case management services including but not limited to; coordinating and facilitating Treatment Team Meetings, support to foster parents, biological families (if appropriate) and the youth. Ensure Private Placement services and provide necessary contacts. For State committed foster children, coordinate services and provide State mandated contact. Provide on-call after hours services as needed for youth and foster families.

Requirements: Bachelor’s Degree in psychology, sociology, social work or related field. Must have at least five year experience working directly with children who have emotional disabilities/SED as a case manager. Valid driver’s license and auto insurance. Must possess strong written and verbal skills. Ability to work some weekends and evening hours (including participation with on-call after hours rotation schedule with other TFC Case Managers).
