Brief Job Description:
Position will be based in Warren County, but travel will be required to Allen, Barren, Butler, Edmonson, Hart, Logan, Metcalfe, Monroe, Simpson and Warren. The minimum educational requirement is a current KY Certified Medical Assistant, Registered Medical Assistant or Licensed Practical Nurse. Applicants need to be highly motivated, independently thinking professionals. Duties include working to decrease emergency department visits and hospital readmissions of patients with chronic diseases through home visiting for care coordination; customized, evidence-based patient education; and social support. The applicant will link patients to medical, social, and environmental services, provide culturally relevant self-management education, provide basic health services to patients, document services in the Electronic Medical Record, maintain a caseload of 3-4 hospital/home visits daily and maintains confidentiality and privacy according to guidelines.
