Title: ACT Team Case Manager

Hours Status: Full Time 
POSITION SUMMARY: The Case Manager will provide support services to participants in ACT program and their families.   Position consists of resource acquisition, linking to services, coordinating care for physical and mental health needs and monitoring changing needs of participant. QUALIFICATIONS:Position requires a Bachelor’s Degree in psychology, social work or behavioral science with one year of case management experience working with individuals with mental health needs in a human service setting. Experience must be post degree. Must have reliable transportation with valid driver’s license and auto insurance. Must possess strong written and verbal skills. Must be willing and able to work flexible hours at times as well as alternating crisis phone responsibilities. Criminal background checks to meet agency standards.
