Sign on bonus is explained in the interview. Travel reimbursement and opportunities to make additional income. Flexible work schedule Monday through Friday, which could include working from home.
Position Summary:
Under direct supervision of PDS supervisor, the PDS support broker is responsible for providing support broker services to participants in the Michelle P Waiver (MPW), Supports for Community Living (SCL), and Acquired Brian Injury (ABI) waivers.
ESSENTIAL FUNCTIONS:
- Conduct MAP-351 assessments and reassessments accurately and timely (complete no earlier than 60 days and upload in MWMA no later than 40 days prior to participant’s ending LOC)
- Assist individuals with PDS enrollment process
- Assist with the development, revision, and monitoring of the person centered service plan
- Conduct a monthly face-to-face visit with participant
- Conduct a quarterly face-to-face visit with representative
- Monitor and document participant’s health, safety and welfare
- Assist with the development, revision, and monitoring of emergency backup plans
- Provide education and information to all participants on self-determination and community resources
- Provide training to all PDS representatives (and employees as needed) on the following: 1) Person Centered Planning; 2) Abuse, Neglect, Fraud, and Exploitation; and, 3) Incident Reporting Procedures
- Serve as case manager to individuals that have chosen the PDS or PDS-blended service model
- Assist participants with recruiting, hiring, training, and managing their employees and negotiating pay rates
- Assist participants with all paperwork related to the PDS process
- Be available to the PDS participant twenty-four (24) hours per day, seven (7) days per week
- Coordinate all services for the individual(s) receiving blended services
- Ensure individuals’ rights are respected and individuals are free of any and all forms of abuse or mal treatment
- Support individuals to maximize personal choice and honor those choices within reason
- Develop person centered plans based on individual preferences and needs using person centered approaches
- Maintain up-to-date Level of Care (LOC) on all participants
- Responsible for transportation to and from home visits and any other facility/agency
- Maintain proper and appropriate documentation of all supports provided in a timely manner
- Having a working knowledge of MPW / SCL / ABI / PDS regulations and maintaining appropriate documentation to meet these guidelines
- Complete billings for appropriate supports provided in a timely manner
- Ensure person centered service plans in MWMA are within regulation guidelines before PA is generated to ensure services are not over maximum limits
- Ensure the maximum billable rate for each service covers the employee’s pay rate-make sure to check the most recent SUTA tax rate
- Ensure the type of services including the scope, amount and duration and goals/objectives being required on the person centered service plan align with the participant’s Map 351
- Document all incidents in a timely manner in MWMA and notify the appropriate parties
- Monitor and ensure PDS participants do not go over monthly approved units
- Ensure monthly contact note is uploaded timely in MWMA
- Train participants and/or representatives how to use Mains’l and EVV app
- Attend all necessary training and updates as needed
- Submit required documentation via MWMA within required timeframe
- Return telephone calls within 24 hours or the next business day
- Review PDS employee timesheets and service documentation to ensure all rules and regulations are followed
- Contact participant, guardian and representative about any Medicaid eligibility issues
- Issue corrective action plans (CAP) as needed
- Assist with obtaining quotes and ordering goods and services as needed
- Maintain communication with all outside agencies involving participant’s services
ADDITIONAL FUNCTIONS:
- Provide crisis intervention, direct support, and transportation to participants as needed
- Perform other duties as assigned by the supervisor
