Brief Job Description:

The minimum requirement is a Bachelor’s Degree in Social Work, Nursing, Early Childhood Education OR a Bachelor’s Degree in Social/Behavioral Science or related field with one (1) year experience performing case management services or a Master’s Degree in Human Services may substitute for the one (1) year of experience. An Associate’s Degree in Early Childhood Development and two (2) years of public health, early childhood or education experience OR Associate’s Degree in Nursing with Licensure in Kentucky or compact state and two (2) years of public health experience will substitute for the required education. Applicants must meet education requirements set forth in 902 KAR 4:120. Duties include conducting social assessments in the clinic and home setting, determining the eligibility for the HANDS program, providing families with child development and parenting information, and linking families to community resources. These positions will also be responsible for completing various forms, monthly reports, and documentation of case records. Other duties as assigned. Applicants must have reliable transportation and a valid driver’s license. Travel will be required to other counties as needed after orientation and training is complete.
