Title: CoC Supported Housing Case Manager
Worksite: Warren Service Center

Hours Status: Full-Time, Mon-Fri, 8a-5p
Pay Range: $15.87 hourly
POSITION SUMMARY:
Advocate and assist individuals with SMI and/or SUD, who are literally homeless, over-come barriers to obtaining and maintaining safe & affordable housing. Assess individuals’ strengths & needs; coordinating supportive services with other program staff; assisting individuals with accessing community resources, and building & maintaining professional relationships with community partners.
*Requires travel within 10-County service area.

QUALIFICATIONS:
- Bachelor’s degree required; preferably in Psychology, Social Work, Sociology or Human Services
- Previous experience providing Case Management services is preferred.
- Valid Driver’s license, insurance and reliable transportation.
