Title: Care Representative

Worksite: Warren Service Center after training this will be a remote position.

County: Warren

Hours Status: Full-Time - 1st shift options are 8:00am to 5:00pm

Starting Pay Range: $15.87 Hourly

 

POSITION SUMMARY:

The Care Representative provides administrative support for a single point of entry into services.

QUALIFICATIONS:

    • Bachelor's Degree in mental health related field.
    • Prefer two years of relevant experience working with individuals who receive services for treatment of a: (mental health disorder, substance abuse disorder or co-occurring disorder)
    • Be eighteen years of age or older.
    • Valid driver’s license and proof of insurance.
    • Must maintain reliable transportation.
    • Once the training is completed this position will primarily be remote with some on-site requirements, therefore a quiet and secure place must be used as this will mostly be the 988 calls.

 

PRIORITY FUNCTIONS:

  1. Follow up on after-hours requests for new service including fax or email referrals.
  2. Maintain accurate and timely schedule master.
  3. Perform telephonic follow-up for missed appointments.
  4. Send mailings when appropriate for client care.
  5. Coordinate with other local agencies to gain referral/client information as appropriate.
  6. Maintain regional resource list to be able to accept calls requesting resources only in lieu of direct services.

 

OTHER ESSENTIAL FUNCTIONS:

 

  1. Accurately document content of telephonic screenings.
  2. Collect information necessary for administrative intake, including verification of Medicaid.
  3. Scan/upload all relevant documents to the EHR.
  4. Utilize motivational techniques to engage individuals missing appointments.

 

ADDITIONAL FUNCTIONS:

    1. Crisis de-escalation and/or coordinating access to crisis unit services if indicated.

2. Promptly answer suicide hotline calls, deescalate and document accordingly
