Title: PDS Case Manager / Support Broker
Worksite: IDDS Administrative Offices – BG (potential to work from home or hybrid)
County: Warren, various surrounding
Hours Status: Full-Time
Pay Range: $17.55 Hourly ** 2,000 Sign-on Bonus **
Brief Job Description:
Provide case management services for individuals with intellectual and developmental disabilities to participants in the Michelle P Waiver (MPW), Supports for Community Living (SCL), Home and Community Based (HCB) and Acquired Brian Injury (ABI) waivers.
- Primary responsibilities include making home visits, completing assessments, creating/updating plans, and monitoring outcomes.
- Travel reimbursement and opportunities to make additional income.
Qualifications include:
- Bachelor’s Degree in the Human Services field is REQUIRED.
- Valid driver’s license and current automobile insurance
- Knowledge of community resources and ability to develop/create appropriate support resources
