Title: Targeted Case Manager – KY MOMS MATR

Worksite: Warren Service Center

County: Warren, regional 10 counties

Hours Status: Full-Time, M-F, 8a-5p

Pay Range: $15.87 hourly

Brief Job Description:

As the case management you will be advocating and assisting pregnant and up to 1-year postpartum clients who have risk factors that might interfere with achieving a healthy pregnancy and infancy. Provide direct services to pregnant and postpartum women, as well as working with communities to increase awareness of pregnancy risk factors and resource availability.

Qualifications:

• Bachelor’s degree in human services (Psychology, Social Work, Sociology, Family studies, Counseling, Public Health, Special Education, Education.)
• One-year post-graduation minimum experience working with SUD adults; or,

one year of case management experience; or,

6 months if possesses master’s degree in approved field.

• Valid driver’s license and current automobile insurance.
