Title: Integrated Care Case Manager, PIPBHC

Worksite: Warren Service Center

County: Warren

Hours Status: Full-Time Hourly, 8a-5p, M-F

Starting Pay Range: $15.87 hourly

Position Summary:
Works with clients to assess, plan, implement, monitor and evaluate actions required to meet the client’s health and human services needed.  Is a member of the Promoting the Integration of Primary and Behavioral HealthCare (PIPBHC) Team.

• Identify client’s needs and barriers to receiving the appropriate care
• Facilitates access to appropriate providers through inquiry, referral, coordination, and timely follow up
• Drafts service plans with clients to establish goals and objectives; reviews case progress and determines case closure
• Works collaboratively with Integrated Care staff and Primary Care Staff to achieve positive client outcomes

Requirements:

• Bachelor’s degree in human services related field.
• Must have a valid driver's license, reliable transportation and present proof of automobile insurance.

Skills:

• Proven work experience in Case Management or related job

• Previous experience with psychological aspects of care
• Problem solving, time management and organizational
