Title: Targeted Case Manager (Adults)
Worksite: WCSC
County: Warren
Hours Status: Full-Time, 8am-5pm with some flexibility
Pay Range: $33,000 annually

POSITION SUMMARY: Case management positions consist of advocating and assisting adult clients experiencing severe mental illness and/or substance use disorders.

ESSENTIAL FUNCTIONS:

• Case Manager Responsibilities:
  • Assist clients with accessing community resources
  • Assess client’s strengths and needs: medical, housing, education, vocation and socialization.
  • Monitor client’s progress
  • Assist with building a network of supports
  • Build professional relationships with community agencies.
  • Provide crisis assistance.
  • Assist client with advocacy efforts with other community services.
  • Maintain appropriate contact with client.
  • Assure client choice and access to community resources.
  • Attend monthly supervision.
  • Return phone calls and emails promptly.
  • Complete and submit documentation by required deadline.
  • Perform other related work as required and assigned.

QUALIFICATIONS:

• BA degree in human services: psychology, social work, sociology, special education or other related behavioral sciences degree.
• One year of experience working with SMI adults or one year of case management experience, post-graduation, or 6 months if individual possesses master’s degree in approved field.
• Valid KY Driver’s license
