Title: Case Manager
Worksite: Park Place Recovery Center
County: Allen
Hours Status: Full-Time, M-F, 8:00am-4:30pm
Pay Range: $15.87 hourly
POSITION SUMMARY:
Provide direct, written, and telephonic contacts with clients and their service providers to improve outcomes and patient satisfaction and to ensure access to care and needed supports.
QUALIFICATIONS:
- BA degree required in human services; psychology, social work, or sociology or be a Kentucky Licensed Practical Nurse.
- Valid driver’s license and insurable driving record required.

SKILLS:
- Previous experience providing case management or care coordination or working with clients with substance use disorders is preferred.
- Must be highly organized, have proficiency in communication technologies (email, cell phone) and personal communication skills.
- Experience with EHR/EMR is desirable.
