Title: Care Coordinator - LPN

Worksite: Warren Service Center

County: Warren

Hours Status: Full-Time, M-F, 8a-4:30p

Pay Range: $22.60 hourly

POSITION SUMMARY:

Provide medical support and care coordination services to individuals and families, integrating physical and behavioral health services. Under the direction and supervision of a registered nurse or licensed provider, the LPN delivers care within the scope of practice defined by the Kentucky Board of Nursing (KBN). This role emphasizes collaboration within an interdisciplinary team to ensure holistic patient-centered care.

QUALIFICATIONS:

    CERTIFICATION/LICENSURE:

• Kentucky LPN licensure (or) license in a State recognized by the Kentucky Nurse Licensure Compact Act.
• CPR current certification.
• Valid Driver’s license and current automobile insurance.

    SKILLS:

• Experience supporting patients with behavioral health and chronic medical conditions.
• Strong communication, documentation, and time-management skills.
• Comfortable working with diverse populations and collaborating across disciplines.
• Competence with electronic documentation, report generation, and care plan follow-through.
