Brief Job Description:
Key responsibilities include: providing intensive case management for individuals with intellectual disabilities referred for crisis services; participating in the DSD regional 24-hour on-call rotation system; facilitating crisis debriefing meetings and providing diligent follow up / monitoring to ensure action steps are completed timely and according to the action plan. Preferred applicants will have crisis prevention/intervention experience and have familiarity with various forms of person-center planning and cross-systems crisis planning.
Qualifications: Bachelor’s Degree in Human Services field and at least three (3) years of experience working with ID/DD population. A minimum of three (3) years case management experience is required.
