Title: Community Living Associate

Worksite: Kinnelwood Residential

County: Warren

Hours Status: Full Time

Pay Range: $13.25 Hourly

 

POSITION SUMMARY:

Under the supervision of the SCL Director and/or Residential Supervisor, provides supervision, assistance and training to individuals living in SCL Staffed residences.

 

ESSENTIAL FUNCTIONS:

1. Responsible for training, instruction, and assistance for individuals to live independently.

2. Provide training and assistance in the areas of routine household care and maintenance, shopping, money management, medication management, socialization, relationship building, and leisure choices.

  1. Assist consumers with personal care needs (bathing, dressing, toileting, eating etc.)
  2. Serve as an advocate for consumers.
  3. Assist individuals with community integration and knowledge of community resources.
  4. Transport individuals to and from various community settings of their choice.
  5. Responsible for ordering/refilling consumer medications as well as proper administration and documentation.
  6. Develop and maintain positive relationships with individuals supported and their families and friends.
  7. Assist individuals with developing and maintaining maximum inclusion within the community.

10. Attend staff meetings, in-services, Plan Of Cares,SIS and Support Team meetings as scheduled.

Monitor residence for cleanliness, organization, safety, etc. and report back to supervisor.

  1. Assist with training new residential staff as needed.

Help Fill in shifts when staff are unavailable.

 

ADDITIONAL FUNCTIONS:

  1. Perform other duties as assigned by supervisor.
  2. Follow Behavior Support Plan and Crisis Prevention Training


QUALIFICATIONS:

High School diploma or G.E.D.

Valid driver’s license and proof of insurance.

Must be willing and able to work flexible hours to include holidays, evenings and weekends.

Attend 40 hours of MR/DD training and all annual updates necessary.
