Hours Status: Full-time/Hourly
Brief Job Description: The SOAR/Path Outreach Case Manager will assist clients by increasing their access to the disability income benefit programs administered by the Social Security Administration for eligible adults who are homeless or who are at risk for homelessness. In addition, the SOAR/PATH Outreach Case Manager will provide community outreach, internal/external staff training, and case management services in the ten county service area. The SOAR/PATH Outreach Case Manager will also ensure data and program compliancy with all state and federal grant guidelines. This position requires a Bachelor’s Degree, preferred candidates will have experience working with individuals who have been diagnosed with a severe mental illness, and or, individuals who are experiencing homelessness. Must have fundamental knowledge of Microsoft Office programs; possess excellent written and verbal communication skills, efficient organizational skills, detail oriented and able to work in a fast paced environment while managing multiple tasks independently. Travel is required in the LifeSkills ten county service area. Applicant must also have a valid driver’s license and reliable transportation with current insurance.
