Title: Community Living Associate
Worksite: DDID Edgewood Residence group home
County: Logan
Hours Status: Part Time, varied shifts
Pay: $13.25 Hourly, eligible for shift differential

POSITION SUMMARY:
Under the supervision of the Edgewood House Manager provides supervision, assistance and training to individuals living in Edgewood group home.


ESSENTIAL FUNCTIONS:

· Responsible for training, instruction, and assistance for individuals to live independently.

· Provide training and assistance in the areas of routine household care and maintenance, shopping, money management, medication management, socialization, relationship building, and leisure choices.

· Assist consumers with personal care needs (bathing, dressing, toileting, eating etc.)

· Serve as an advocate for consumers.

· Assist individuals with community integration and knowledge of community resources.

· Transport individuals to and from various community settings of their choice.

· Responsible for ordering/refilling consumer medications as well as proper administration and documentation.

· Develop and maintain positive relationships with individuals supported and their families and friends.

· Assist individuals with developing and maintaining maximum inclusion within the community.

· Attend staff meetings, in-services, Plan of Cares, SIS and Support Team meetings as scheduled.

· Responsible for residence cleanliness, organization, safety, etc. and report back to supervisor.

· Assist with training new residential staff as needed.

· Help Fill in shifts when staff are unavailable.

ADDITIONAL FUNCTIONS:

· Perform other duties as assigned by supervisor.

· Follow Behavior Support Plan and Crisis Prevention Training

QUALIFICATIONS:

· High School diploma or G.E.D.

· Valid driver’s license and proof of insurance.

· Must be willing and able to work flexible hours to include holidays, evenings and weekends.
